Medical Treatment Consent

I hereby consent that my child can receive any necessary medical treatment for any condition or injury suffered while attending the 2025 Baron Basketball Camp. I understand that the camp does not carry medical or accident insurance on behalf of any camper and that I will be responsible for any expenses incurred on his/her behalf in connection with such treatment. I release the camp and its workers from all claims and liabilities of injuries sustained by my son/daughter. I confirm that my son/daughter is physically fit to participate in the camp and agree that he/she is adequately covered by medical insurance.

Tuesday, May 13th at 6:00 PM

Event Policy: All sales final. No refunds or Exchanges - This event has a no refund policy.

No refunds, No re-entry, all sales are final. Buckeye Valley High School is a tobacco-, drug-, and alcohol-free campus. Absolutely no smoking or vaping on school property. No pets allowed with the exception of a working service animal. No outside food or drinks are permitted at any Buckeye Valley athletic event. Spectators cannot bring any ball or other recreational equipment into an athletic facility for personal use. All spectators are expected to observe the athletic event from the bleachers, please do not stand or loiter. Let the players play, let the coaches coach, let the officials officiate, and let the spectators be positive. By purchasing or being in possession of these tickets you understand and agree to these policies

Monday, June 16th – Wednesday, June 18th

This camp is for rising 2nd through 6th graders (Grade for the 2025 - 26 School year) 

It will be held June 16th - 18th from 9 am to Noon at the Baron Baseball Field. 

T-Shirts are ordered two weeks prior to camp; if registering after, you will get a shirt, it just won't be at the camp. It will be delivered to you when it arrives.

Event Policy: This event has a no refund policy.

Camp will be held Monday, June 9th, Wednesday, June 11th, and Friday, June 13th. Incase of rain the rain date is Monday June 16th. 

Camp is open to ages PreK-8th grade, boys and girls.

Grades PreK- 1st Grade is 6 - 7:30 pm 

Grades 2nd - 8th grade is 6 - 8 PM

Waiver - Exclusion Clause; Consent 

WAIVER - EXCLUSION CLAUSE I, the undersigned parent or guardian, in enrolling my child in Baron Soccer Camp, understand that he/she in attending any soccer program and using the facilities does so at his/her own risk. Baron Soccer Camp and their representatives, Buckeye Valley School District, and their employees and representatives shall not be liable for any damage whatsoever arising from any personal injury or property loss sustained by a participant and his/her family in or about any programs on the premises, inclusive of transportation. Participants and their parents/guardians assume full responsibility for all injuries and damages which may occur in or about any programs on the premises and he/she/parent/guardian/family member does hereby fully and forever release, discharge, and hold harmless Baron Soccer Camp and their representatives, Buckeye Valley School District, and their employees and representatives, all associated facilities, and its employees and agents from any and all claims, demands, rights of action, present or future resulting from or arising out of any person’s participation in any programs or use of its facilities.

CONSENT I, the undersigned parent or guardian, do hereby grant authority to the staff of the Baron Soccer Camp to render judgment concerning medical assistance or hospital care in the event of an accident or illness during my absence.
